County Identification Tag Request

Training Authorization Form
This form, signed by an authorized representative of your department, must be filled out and brought with you on the first night any OFPC sponsored course.

 

Firefighter Background Check
All new firefighters must complete a Firefighter Background Check as established in Executive Law §837. This form must be mailed, faxed, or hand-delivered to the St. Lawrence County Sheriffs Office. Email transmission of this form is not permissible.
