Early Intervention Program

Program Goal: To support and promote the development of those infants and toddlers who have special needs related to developmental delays and to enhance the capacity of their families to meet their needs.

Eligibility: Children may receive Early Intervention Program (EI) services if they are age birth to three and meet eligibility criteria for developmental delay, or a diagnosed physical or mental condition that has a high probability of resulting in a developmental delay (in one or more of the developmental areas of: physical; cognitive; communication; social-emotional or adaptive).

Description: Anyone can refer a child under three years of age to the EI Program. EI services available to eligible children and their families may include:

  • Service Coordination
  • Speech/Language Pathology
  • Audiology
  • Special Instruction
  • Physical Therapy
  • Occupational Therapy
  • Social Work Services
  • Family Training or Counseling
  • Nursing Services
  • Nutrition Services
  • Other Related Services

 

These services are typically provided in the child’s home or at another location (such as a child care site) where the child typically spends his or her time.

Child Find

Provides developmental assessments of children age birth to three.  Educate parents regarding child developmental stages.  Referrals made to other providers as needed.


Children and Youth with Special Health Care Needs

Program Goal: Provide resources and referrals for children in the community to ensure access to health care, insurance, information and support.

Eligibility: Children from birth to 21 years who have (or may have) a serious or chronic physical, developmental, behavioral or emotional condition.

Description: This program provides assistance in locating a physician or other health care provider to ensure each child has a medical home. The program may also provide assistance in obtaining insurance and/or other programs that help to cover health care costs – i.e. Medicaid, Child Health Plus. Supportive measures as well as medical information may be provided.

Maternal Child Health Program

Description: The Maternal Child Health Program offers a one time home visit for mom and baby anytime following hospital discharge.

Services offered include:

  • Assistance with breastfeeding
  • Head-to-toe assessment of baby
  • Education on postpartum and newborn care
  • Education on postpartum self-care
  • Referral to services in the community if needed

Preschool Special Education Program

We facilitate services for children ages 3 to 5 years who have special needs.

This program offers evaluations and a variety of services to help maximize the child’s developmental potential. If you suspect that your child has a developmental delay or disability, contact your local school district’s Committee on Preschool Special Education to make a referral.

Women, Infants and Children (WIC) Services

WIC is a Special Supplemental Nutrition Program for pregnant, breastfeeding, and postpartum women, as well as for infants and children under the age of 5.
