Adult Single Point of Access (ASPOA) provides access to intensive community based Health Home Care Management, Assertive Community Treatment (ACT), Peer Support and/or Housing services for adults with severe mental illness. These services are intended for individuals who are at high risk of psychiatric hospitalization and/or who are unable to maintain community based linkages and supports, such as: psychiatric treatment, medication management, medical treatment, housing, substance abuse treatment, legal, and other social supports. The goal of ASPOA is to have the individual receive the right amount of support at the right time.
