Notice of Potential COVID-19 Exposure


Where: Canton Pizza Hut, 5933 US Highway 11 University Shopping Ctr, Canton, NY 13617


Date: Saturday, November 7th 2020


Time: 12:00 PM – 1:00 PM


Individuals who dined at Pizza Hut at the above location during the day and time listed have a
potential risk of exposure to COVID-19.


If you believe you were present at this location on Saturday, November 7th between 12:00
PM – 1:00 PM it is recommended that you be tested for COVID-19 and monitor how you feel.
Please monitor your temperature twice a day for 14 days. Call your healthcare provider for
further guidance if you develop the following symptoms: fever, chills, cough, shortness of
breath or difficulty breathing, fatigue, muscles or body aches, headache, new loss of taste or
smell, sore throat, congestion or runny nose, nausea or vomiting, and diarrhea. Tell your
provider that you may have been exposed to COVID-19.
